*Promotional Offer — Terms & Eligibility

This promotional offer is valid through June 30, 2026 and is available to new Fusion Medical Billing & Consulting clients only. To be eligible, the following conditions must be met:

• Applicant must be a first-time Fusion client with no prior service agreement, active or terminated, within the preceding 24 months.

• Offer applies to Full Revenue Cycle Management / Credentialing / Practice Consulting only and may not be applied to add-on services, bundled packages, or services not expressly included in the promotional terms.

• Eligible practices must execute a signed Fusion service agreement for a minimum of one year on or before the offer expiration date. Verbal commitments, letters of intent, or unsigned agreements do not qualify.

• Offer is non-transferable and may not be assigned to any third party, affiliate, or related practice entity.

• This offer may not be combined with any other Fusion promotional pricing, referral incentive, or discount program unless explicitly authorized in writing by Fusion management.

• Fusion Medical Billing & Consulting reserves the right to verify eligibility prior to applying promotional pricing and to disqualify applicants who do not meet all stated requirements.

• Fusion reserves the right to modify, suspend, or discontinue this offer at any time prior to the expiration date with reasonable notice. Offers already confirmed in a signed agreement will be honored at the agreed rate.

• Promotional pricing applies to the initial contract term only. Standard rates apply upon renewal unless otherwise specified in the service agreement.
